Why Join A Circle of Love Home Care? • Flexible, contract-based RN role — no hospital shifts • Per-client pay ($60 per client) • Ideal for part-time, semi-retired, or supplemental income RNs • $150 sign-on bonus after first completed assessment • $100 client referral bonus + RN referral incentives • Supportive, growing home-care organization • Make a real impact in patients’ lives — without burnout • Mileage reimbursement available for multi-client days Location: Rockdale, Walton, Jackson, Fulton, Cobb, Dekalb, Gwinnett, Douglas, Clayton, Clarke, Fayette, Henry, Paulding, Newton, and Forsyth Job Responsibilities: • Perform initial and ongoing assessments of clients’ physical and emotional health. • Develop, implement, and regularly update client care plans in coordination with the healthcare team. • Administer medications, treatments, and interventions as prescribed by physicians. • Monitor and document client conditions, reporting any changes to the healthcare provider and adjusting care plans as necessary. • Educate clients and their families about health conditions, treatments, and best practices for ongoing care at home. • Conduct supervisory visits to evaluate the quality of care being provided by other staff members, ensuring compliance with care plans and safety standards. • Communicate with physicians, family members, and care team to ensure all client needs are met. • Provide emotional support to clients and their families, addressing concerns and helping them navigate healthcare options. • Assist with transitions of care, such as discharge planning and referrals to other services when needed. • Participate in case conferences, providing input on client progress and future care strategies. • Ensure proper documentation of all care provided, including assessments, treatments, and communications with other healthcare providers. • Adhere to agency policies, procedures, and regulatory requirements. • Perform other related duties as needed and assigned. What We Prefer: • Willing to travel inside and outside of the perimeter • Excellent verbal and written communication skills. • Strong organizational skills and attention to detail. • Ability to work both independently and collaboratively within a team. • Strong clinical judgment and problem-solving abilities. • Proficiency in performing nursing assessments and interventions. • Ability to prioritize tasks and manage time effectively. • Proficient with Microsoft Office Suite and electronic medical records (EMR) systems. • Be familiar with all applicable local, state, and federal laws and regulations concerning the operation and provision of home care services. • Be able to communicate and understand return communication effectively in exchanges between the consumer, family representatives, and other providers, including the use of appropriate translator services as needed. Education and Experience: • Be at least twenty-one (21) years of age and possess a high school diploma or GED. • Current and active RN license in the state of practice (Georgia). • Minimum of 1-2 years of nursing experience, preferably in home care or a similar setting. • CPR and First Aid certification. • Annual TB test (required). • Valid driver’s license (required).
