
ACT Team Registered Nurse in Alpine, NJ
Job Description
Annual Salary: $90k – $109k (varies depending on years of experience)
A community behavioral health organization is seeking a full-time Registered Nurse to join a mobile, multidisciplinary treatment team that delivers intensive services to adults living with serious mental health conditions. This model of care focuses on providing consistent, community-based support for individuals who benefit from a highly coordinated, team-led approach.
The RN supports clients in various community settings by completing nursing assessments, monitoring physical health needs, coordinating with external medical providers, and administering prescribed medications—including long-acting injectable treatments. The nurse serves as a key clinical resource and collaborates with a dedicated team that includes clinicians, specialists, care managers, and psychiatric providers.
Key Responsibilities
• Perform comprehensive nursing assessments and ongoing monitoring of medical and behavioral health needs.
• Administer medications as ordered, with a strong emphasis on long-acting injectable (LAI) treatments.
• Coordinate care with external healthcare providers, hospitals, and clinics to support continuity of treatment.
• Manage medication-related tasks such as pillbox organization for selected clients.
• Conduct community visits, including home-based services and meetings in public settings.
• Participate in daily team meetings, case conferences, and treatment planning sessions.
• Document all nursing services and client interactions accurately and in a timely manner.
• Provide education to clients regarding medication adherence, wellness, and symptom management.
• Monitor vitals when clinically indicated; perform limited point-of-care testing (e.g., finger sticks as needed).
• Engage collaboratively with the multidisciplinary team—each team member contributes to every client’s care plan.
• Work independently in the field, with paired visits for new clients or when safety considerations require it.
• Support a person-centered, recovery-focused approach to mental health treatment.
Qualifications
• Current Registered Nurse license in New York State
• Experience with behavioral health, serious mental illness, or community-based care preferred
• Willingness to perform field-based work daily
• Excellent communication and organizational skills
• Commitment to recovery-oriented and person-centered care
