Admission Hospice RN Registered Nurse in Bethlehem, PA

Job description: We are seeking a compassionate and detail-oriented Hospice Admission Nurse (RN) to join our dedicated team. This role is vital in guiding patients and families through one of life’s most meaningful transitions. Position Summary The Hospice Admission Nurse is responsible for conducting comprehensive admission assessments, educating patients and families about hospice services, and coordinating the initiation of care. This nurse serves as a compassionate advocate, ensuring a smooth and supportive transition into hospice care.
