Job Description
Position: Admissions RN
Salary: $42-$55/hr
Schedule: Monday - Friday, 12:00 pm - 8:00 pm
Shift differential for PM ($1.25/hr.), NOC ($1.50/hr.), and Weekend ($3.00/hr.)

Who you are:
Current licensure by the California Board of Registered Nursing
Current CPR certification
Two years in long-term care with prior charge nurse experience preferred
Experience with wound care and admissions strongly preferred
Knowledge of CMS regulations and long-term care standards
Strong assessment, organizational, and communications skills
Ability to work independently and as part of an interdisciplinary team
Job Summary:
Coordinates and completes clinical admissions for new residents in a timely and accurate manner.
Essential Job Duties:
Perform comprehensive nursing assessments upon admission
Review hospital records, discharge summaries, medication lists, and physician orders
Initiate and document care plans based on resident needs
Communicate effectively with physicians, interdisciplinary team members, residents, and families
Ensure all admission documentation meets regulatory and facility standards
Perform wound assessments, treatments, and dressing changes per physician orders
Ensure compliance with infection control, safety, and quality assurance standards
Maintain accurate and timely documentation in the electronic medical record (EMR)
Participate in care plan meetings and interdisciplinary rounds as required
Assist with audits, surveys, and regulatory compliance activities
Support quality improvement initiatives related to admissions and wound care
