Home Health Admission Registered Nurse (RN)
in Pueblo, Colorado
Status: Full Time
Salary: $80,000 per year, which includes potential bonuses
Shifts: Full Time; Monday-Friday, 8a-5p, with on-call rotation

As a Home Health Admission RN, here’s a big-picture view of what you’ll do:
• Complete initial and ongoing comprehensive assessments.
• Coordinate for patient/client services.
• Provide care according to the patient’s/client’s plan of care/service plan.
• Participate with the patient/client in clarifying mutually agreed upon goals and incorporate these goals into the plan of care/service plan.
• Conduct home visits as assigned, assessing the patient’s condition, progress toward goals, providing patient care, teaching and other skilled interventions as ordered.
• Develop a care plan in collaboration with Case Manager, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions, including the patient and family in the planning process.
A few must-haves for Home Health Registered Nurses:
• Graduate of an accredited nursing program and active RN license in Colorado
• Minimum of one (1) year of nursing experience, ideally in home healthcare
• OASIS experience, preferred.
• CPR Certification (demonstration course required)
• Pass a background check and drug test (we are required to test for THC)
• Able to work independently and provide effective communication to patient/family, team members, and other health care professionals.
• Knowledge of state, federal, and licensure regulations.
• No Covid Vaccination Requirements
