Job Summary: Under the supervision of the Team/Program Manager, the field clinician is responsible for managing a caseload of patients from admission to transition into the community. This involves interdisciplinary support and referrals when appropriate. The position coordinates interdisciplinary care, monitors and adjusts treatment to achieve positive outcomes, and identifies opportunities for process improvement. Responsibilities are carried out in accordance with organizational policies, procedures, and standards aligned with Federal, State, and accreditation regulations.
Qualifications: Equivalent education required for RI licensure; a Bachelors Degree in a relevant discipline is strongly preferred.
