Title: Admissions RN
Pay: $42.34 to $50.81 per hour
Bonus: $3,600
Schedule: Saturday, Sunday, & Monday. Shift time options of either 10am-6pm, or 12pm-8pm
Location: Out in the community. Louisville, Longmont, Boulder, Ft. Collins, Denver, Parker.
JOB SUMMARY:
The Admissions Nurse is responsible for admitting patients to TRU Community Care and the Home Health programs in a timely manner upon patient, family, or referral source request. This role will have you in the community - in homes and facilities in the surrounding area. The Admissions Nurse conducts the intake and completes the admission process in a manner that promotes an interdisciplinary team approach to care, respects patients/families, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU Community Care.
ESSENTIAL DUTIES
• Works with the Admissions Coordinator to assure a timely response to referral sources and patients/families
• Provides clinical documentation of services that demonstrates appropriate and high quality admissions procedures including but not limited to informed consent for clinical care and compliance with Medicare conditions of participation
• Identifies needs of hospitals and makes recommendations regarding meeting those needs and offering solutions
• Conveys a positive and professional image to referral sources and the community at large
• Demonstrates excellent assessment skills in determining eligibility and General InPatient levels of care
• Incorporates patient/family preferences and goals within the plan of care
• Completes documentation in an accurate and complete manner as per agency standard and in compliance with regulatory standards
• Provides patient and family education as appropriate; areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, disease process, death and dying
EDUCATION AND QUALIFICATIONS
• Must possess a current Colorado RN license
• Two years of experience as an RN
• Hospice or home health experience preferred
• Must successfully pass agency required background checks and drug screening
• Requires travel to multiple work locations
• Must possess CPR certification or be able to obtain within 60 days of hire
• Must comply with agency health screening and immunization requirements
