Nurse Practitioner – Post-Discharge In-Home Care (Crossville, TN & Surrounding Areas)

Segue Health, a faith-based medical practice, is seeking an experienced and compassionate Nurse Practitioner to join our team serving patients in Crossville, Cumberland County, and surrounding communities. This role focuses on providing in-home post-discharge care management and medical treatment for patients transitioning from acute care facilities back to their homes.
Key Responsibilities
Conduct in-home patient visits to assess clinical status and provide post-discharge medical care.
Review and interpret discharge summaries and progress notes.
Collaborate closely with home health professionals and other members of the care team to ensure seamless coordination of care.
Provide medical consultations, prescribe medications when appropriate, and oversee ongoing care plans to promote recovery and reduce readmissions.
Qualifications
Current Tennessee medical license and active Medicare provider number required.
Nurse Practitioner certification in good standing.
Ability to travel within a 35-mile radius from home to see patients throughout Crossville and surrounding communities.
Strong communication, clinical judgment, and documentation skills.
A compassionate, faith-driven approach to patient care.
Compensation:
Base Salary: $115,000 – $135,000 (commensurate with experience)
Quarterly Performance Bonuses available
