Seeking compassionate NP or PA for palliative care and Home-Based Primacy Care in beautiful Helena, MT—make a real difference in end-of-life care!
About the Role
Join our dedicated team in providing high-quality palliative care and Home-Based Primary Care (HBPC) services to patients with serious, advanced, or chronic illnesses in Helena and surrounding areas. As a Palliative Care provider, you will deliver expert symptom management, supportive care, and coordination to improve quality of life for patients and their families. This role combines clinical autonomy with interdisciplinary collaboration in a meaningful, rewarding environment focused on dignity, comfort, and holistic support during challenging times.
Schedule/Clinic Hours- The office is open Monday- Friday 8:30am to 5pm. Schedule is flexible. On-Call required.
Key Responsibilities
• Provide palliative care consultations and HBPC services in compliance with organizational policies, state regulations, and under the supervision of a collaborating physician.
• Perform comprehensive history and physical examinations, evaluate medical conditions, order and interpret diagnostic tests/laboratory results, and document findings accurately in the patient's health record.
• Manage advanced illnesses by developing individualized care plans, prescribing medications and treatments, obtaining consultations/referrals, and coordinating transitions to hospice when appropriate.
• Educate patients and families on disease progression, care options, symptom management, and end-of-life planning, including assistance with advance directives.
• Collaborate with the palliative care interdisciplinary team to ensure appropriate symptom control, review clinical information, and oversee patient care.
• Serve as a resource for education and consultation to team members and community healthcare providers.
• Complete timely and accurate documentation, including clinical notes, assessments, medical orders, and ensure proper coding for all patient visits.
• Build and maintain relationships with staff, patients, families, and community resources to support program growth.
• Enroll eligible patients in chronic care management programs.
• Consult with the collaborating physician for cases outside standard protocols.
• Perform other duties as assigned to support team and patient needs.
Job Conditions
• Full-time position based in Helena, MT, with a hybrid work environment combining office-based, remote, and home/facility visits.
• Travel required to patient homes, facilities, or other care settings within the service area.
• Work may involve emotionally demanding situations; strong support provided through team collaboration and resources.
Qualifications
• Graduate of an accredited school of nursing and an accredited Nurse Practitioner program.
• Current, active Registered Nurse and Nurse Practitioner license in the State of Montana.
• Current, active PA license in the State of Montana
• Current CPR certification and DEA license.
• Minimum of one (1) year NP/PA experience AND one (1) year experience in hospice, palliative care, family practice, or related field.
• Experience with chronic care management services preferred.
• Active Montana Driver’s License with reliable, insured automobile in good working order.
• Excellent clinical observation, assessment, verbal/written communication, problem-solving, and interpersonal skills.
• Compassionate, empathetic approach with comfort discussing serious illness, goals of care, and end-of-life issues.
• Ability to work independently and collaboratively in a team-oriented environment.
