You will coordinate care with the patient, their family, and the hospice care team, ensuring every individual receives the quality care they deserve. This position is full-time, covering call hours from 5 pm to 8 am, Monday through Thursday, concluding Friday morning at 8 am. You will receive ample support from our 24/7 RN-staffed Triage Department and your local hospice team. Qualifications: • A heart to serve patients and families, with a passion for delivering best-in-class care. • Education: Graduate from an accredited nursing program and possess a current state license as a registered nurse. • Experience: A minimum of 2 years of nursing experience in a clinical care setting, with hospice experience preferred. • Required: Reliable transportation; ability to sit, stand, bend, move intermittently, and lift at least 80-100 lbs effectively.
