Join Our Team as an After Hours Hospice Registered Nurse / RN! Location: Middlebury, CT Position Type: Full-Time Remote/Virtual Position: No Coverage Area: New Haven and Fairfield County Salary: $90,000 - $105,000 / year Schedule: 7 on / 7 off 5pm - 8am Sign-On Bonus: $10,000 available! What We’re Looking For: • You hold current licensure as a Registered Nurse in the applicable state, with an associate degree preferred. CPR certification is suggested, and experience in hospice, home health, medical-surgical, or specific nursing fields is advantageous. • You are willing to participate in an on-call rotation and have reliable transportation, a valid driver's license, and state-required liability auto insurance. • You possess strong communication and organizational skills, and are proficient in Microsoft Office Suite and EMR systems. • You are physically capable of performing necessary tasks, including lifting patients with assistance and transporting supplies. Your Responsibilities Will Include: • Conducting comprehensive nursing assessments and collaborating with patients, families, and the healthcare team to create and evaluate tailored care plans. • Maintaining accurate documentation and ensuring that care plans reflect current patient needs, enhancing communication with the care team. • Providing education and emotional support to patients and caregivers, addressing their unique requirements. • If serving as a Care Manager, coordinating all aspects of patient care, including scheduling and medication management. • In an On-Call capacity, responding to triage calls and performing visits and assessments as necessary.
