
After Hours RN in Coldwater, OH
Job Description
Location: Western & Central Ohio
Schedule: Monday–Friday, 4:00PM - 12:00AM
Weekend Rotation: Every 4th weekend (Friday & Monday off after worked weekend)
Holiday Rotation: 2 holidays per year
Pay: $40.00 – $45.00/hour (based on education, experience, and certifications)
Hours: Full-time (40/week)
Sign-On Bonus: $5,000 (for Full-Time position)
Summary of Position: The After-Hours Nurse is responsible for direct patient care after business hours.
Qualifications:
• Current Ohio RN license
• Minimum 2 years RN experience (Med/Surg, Oncology, or Community Health preferred; hospice a plus)
• Valid driver’s license and reliable transportation
• Strong communication and time management skills
• Ability to work independently
Responsibilities of Position:
• Provide skilled, compassionate nursing care including symptom management, emotional support, patient/family education, and timely response to needs
• Deliver care through routine home visits, end-of-life/continuous care, referrals, admissions, care center support, and chart reviews
• Perform thorough initial nursing assessments and monitor patient condition
• Explain hospice services, insurance benefits (Medicare, Medicaid, private), palliative care interventions, and appropriate use of ABNs
• Collaborate with physicians and Medical Directors and obtain/enter orders as appropriate
• Order and coordinate Durable Medical Equipment (DME) to meet patient needs
• Facilitate continuity of care, including home-to-inpatient transitions (hospital, nursing home)
• Complete accurate, timely documentation in compliance with agency, accreditation, and regulatory standards
• Maintain clinical competency and participate in weekend rotations or other duties as assigned
