We are excited to welcome a compassionate Afterhours Registered Nurse Care Coordinator dedicated to providing exceptional hospice care during non-standard hours. In this vital role, you will ensure continuous quality care and clinical oversight for patients and families after hours, including nights and weekends.
Role Overview
The Afterhours Care Coordinator RN will assess and coordinate the needs of patients and their families during after-hours shifts, while ensuring primary clinical oversight for designated patients. Your responsibilities include conducting routine visits, managing admissions, handling urgent calls, and fulfilling care plans tailored to each patient's needs. You will actively contribute to the Interdisciplinary Group (IDG) to promote coordination among physicians, families, and the hospice team, while also taking on administrative tasks to maintain efficient workflow and compliance.
Key Responsibilities
• Deliver high-quality hospice services to patients and families who qualify.
• Provide in-person patient care during on-call, weekend, and after-hours shifts, which may involve assessments, admissions, and patient discharges.
• Evaluate patients' physical, emotional, psychosocial, and spiritual needs through initial and ongoing assessments.
• Facilitate the IDG process with effective chart reviews and updates to care plans.
• Develop and modify the Hospice Plan of Care in partnership with physicians and team members.
• Guide patients and families through information on disease progression, symptom management, medications, and hospice principles.
• Communicate significant changes in patient conditions swiftly to relevant healthcare professionals.
• Ensure accurate documentation of clinical activities, including assessments and care plans.
• Handle visit requests based on triage recommendations, including rescheduling and reassignment.
• Assist in scheduling adjustments, service codes, and clinician assignments.
• Support compliance and administrative tasks including workflow audits and documentation preparation for surveys.
• Participate in Quality Assurance Performance Improvement (QAPI) initiatives.
• Adhere strictly to infection control standards and safety protocols.
• Offer leadership to field staff and conduct supervisory visits as necessary.
• Engage in meetings, after-hours communication updates, and training sessions.
• Maintain expertise in pain and symptom management for hospice and end-of-life care.
• Foster a culture of compassion and teamwork while upholding organizational values.
• Perform additional duties as required.
Qualifications
• Current RN license in the respective state.
• Graduate of an accredited nursing program.
• At least one year of experience as an RN in acute care, oncology, long-term care, hospice, or home health.
• Compliance with state-specific requirements.
Required Licenses and Certifications
• Active RN license.
• CPR certification (in-person).
• Valid driver's license, reliable transportation, and current auto insurance.
Essential Skills and Competencies
• In-depth knowledge of hospice care principles, Medicare/Medicaid regulations, and best practices.
• Ability to conduct comprehensive patient assessments and tailor care plans accordingly.
• Strong communication skills with patients, families, medical staff, and team members.
• Excellent time management and prioritization abilities in a dynamic environment.
• Familiarity with EMR systems (HomeCare HomeBase preferred), Microsoft Office, and mobile technology.
• Commitment to maintaining confidentiality and achieving quality outcomes.
• Leadership capabilities to motivate and guide field staff.
• Ability to accurately manage documentation and compliance tasks.
