We are actively seeking a compassionate Afterhours Registered Nurse Care Coordinator to provide exceptional hospice care outside of standard business hours. In this fulfilling role, you will ensure seamless coverage, continuity of care, and robust clinical management for patients and their families during nights, weekends, and on-call hours.

About the Role

The Afterhours Care Coordinator RN will assess and coordinate patient and family needs during after-hours shifts, maintaining primary clinical oversight for assigned patients. Your responsibilities will include routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s care plan. You will be an integral member of the Interdisciplinary Group (IDG), ensuring effective coordination between physicians, families, and the hospice team. Clinical administrative tasks, workflow audits, and survey readiness support are crucial components of this role.

Essential Functions
• Deliver high-quality hospice services to eligible patients and their families.
• Provide in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges.
• Conduct comprehensive assessments of physical, emotional, psychosocial, and spiritual needs.
• Support the IDG process through chart reviews, plan of care updates, and effective interdisciplinary communication.
• Collaborate with physicians and IDG members to develop and revise the Hospice Plan of Care.
• Educate patients and families regarding disease progression, symptom management, medications, and the hospice philosophy.
• Promptly communicate significant changes in patient condition to hospice and attending physicians.
• Maintain accurate and timely clinical documentation, including assessments, care plans, and notes.
• Assist with rescheduled, missed, or reassigned visits based on triage direction.
• Engage in scheduling modifications, including service codes and clinician assignments.
• Support administrative activities, including workflow audits, survey readiness tasks, and documentation collection.
• Participate in TPE reviews, QAPI efforts, and oversee regulatory compliance.
• Ensure adherence to infection control standards and universal precautions.
• Provide guidance and support to field staff, conducting supervisory visits when appropriate.
• Participate in required meetings, after-hours updates, and training sessions.
• Maintain expertise in pain and symptom management for hospice and end-of-life care.

• Perform other assigned duties as needed.

About You:
• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• At least one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Licenses and Certifications:
• Active RN license.
• CPR certification (in-person).
• Valid driver's license, reliable transportation, and current auto insurance.

Skills and Competencies:
• Strong knowledge of hospice principles, Medicare/Medicaid regulations, and best practices.
• Ability to conduct thorough patient assessments and create individualized care plans.
• Effective communication skills with patients, families, physicians, and interdisciplinary team members.
• Proficient time-management and prioritization skills in a fast-paced environment.
• Proficient with EMR systems (HomeCare HomeBase preferred), Microsoft Office, and mobile technologies.
• Commitment to confidentiality, integrity, and quality outcomes.
• Leadership abilities to guide and support field staff.
• Capability to manage administrative, documentation, and compliance responsibilities accurately.
