Position Overview
As an Afterhours RN Care Manager, you will be pivotal in assessing and addressing the needs of patients and families during after-hours shifts. Your work will include conducting assessments, managing urgent care visits, and facilitating effective communication among the interdisciplinary team (IDG). You will uphold high standards of clinical management and continuity of care, ensuring that every patient receives the support they need when it matters most.
Key Responsibilities
• Deliver hospice services to eligible patients and families with compassion and expertise.
• Provide direct patient care during after-hours shifts, managing assessments, admissions, end-of-life visits, and discharges.
• Conduct comprehensive evaluations of patients' physical, emotional, psychosocial, and spiritual needs.
• Collaborate with IDG members and physicians to create and update individualized Hospice Plans of Care.
• Educate patients and families on symptom management, medication usage, and the philosophy of hospice care.
• Quickly communicate any significant changes in patient conditions to the appropriate medical personnel.
• Maintain accurate and timely clinical documentation following all care interactions.
• Assist in rescheduling missed visits and managing clinician assignments effectively.
• Support administrative and compliance tasks, including regulatory activities and workflow audits.
• Provide leadership and mentorship to field staff when necessary.
• Engage in required meetings, after-hours updates, and ongoing training opportunities.
• Demonstrate expertise in pain and symptom management for hospice patients.
• Promote the values of our organization while fostering a culture of professionalism and teamwork.
• Carry out additional duties as needed.
Qualifications
• Current RN license in your state.
• Graduate from an accredited nursing program.
• A minimum of one year of RN experience in hospice, acute care, oncology, or home health settings.
• Must meet any state-specific requirements.
Required Certifications
• Active RN license.
• Current CPR certification (in-person).
• Valid driver's license with reliable transportation and auto insurance.
Desired Skills and Attributes
• Solid understanding of hospice care principles, Medicare/Medicaid regulations, and best practices.
• Strong assessment and care planning abilities.
• Excellent communication skills across diverse populations.
• Effective time management in fast-paced environments.
• Familiarity with EMR systems, especially HomeCare HomeBase, and proficiency with Microsoft Office.
• Strong attention to confidentiality and integrity in all responsibilities.
• Leadership abilities to guide and support field staff.
• Accuracy in managing documentation and compliance efforts.
