We are looking for a compassionate Afterhours Registered Nurse Care Manager dedicated to providing exceptional hospice care during evening, weekend, and on-call hours. This critical role guarantees comprehensive care coordination and clinical oversight for patients and their families outside of standard business hours.
About the Role
The Afterhours Care Manager RN will assess and coordinate the needs of patients and families during after-hours shifts, ensuring consistent coverage and clinical management. Responsibilities include in-person visits, admissions, urgent after-hours calls, and other tasks outlined in the patient’s care plan. As a vital member of the Interdisciplinary Group (IDG), you will facilitate communication among physicians, families, and the hospice team, while also supporting administrative duties and compliance activities.
Essential Functions
• Deliver hospice services to eligible patients and families with compassion and expertise.
• Provide direct patient care during on-call, weekend, and after-hours shifts, including assessments and urgent needs.
• Conduct ongoing evaluations of physical, emotional, psychosocial, and spiritual requirements.
• Contribute to the IDG process through chart reviews and updating care plans.
• Assist in developing and updating the Hospice Plan of Care alongside physicians and IDG members.
• Educate families on disease progression, symptom management, and hospice philosophy.
• Communicate significant changes in patient conditions to physicians promptly.
• Maintain accurate and timely clinical documentation, including assessments and care plans.
• Support with scheduling modifications and service code adjustments based on triage direction.
• Participate in administrative and compliance activities, including audits and documentation collection.
• Ensure adherence to infection control standards and universal precautions.
• Offer guidance and leadership to field staff during supervisory visits.
• Engage in required meetings and training sessions.
• Stay proficient in pain and symptom management relevant to hospice and end-of-life care.

About You
Education and Experience
• Current RN license in the relevant state.
• Graduate of an accredited nursing program.
• At least one year of RN experience in acute care, oncology, hospice, or home health.
• Fulfillment of any state-specific requirements.
Licenses and Certifications
• Active RN license.
• In-person CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
Skills and Competencies
• Strong understanding of hospice principles and Medicare/Medicaid regulations.
• Ability to perform thorough patient assessments and create individualized care plans.
• Effective communication with patients, families, and interdisciplinary team members.
• Excellent time-management and prioritization abilities.
• Proficiency with EMR systems and Microsoft Office.
• Integrity in maintaining confidentiality and supporting quality outcomes.
• Leadership capabilities to mentor field staff.
• Ability to handle administrative and compliance tasks accurately.
