As an Afterhours Registered Nurse Care Manager, you will play a vital role in delivering compassionate hospice care during nights, weekends, and on-call hours. This position guarantees continuous care and robust clinical management for patients and families outside of standard business hours.

Key Responsibilities:
• Deliver high-quality hospice services to eligible patients and their families.
• Provide direct in-person care during after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges.
• Conduct initial and continuous assessments addressing physical, emotional, psychosocial, and spiritual needs.
• Enhance collaboration within the Interdisciplinary Group (IDG) through chart reviews, care plan updates, and communication with team members.
• Develop and refine the Hospice Plan of Care alongside physicians and IDG members.
• Educate patients and families about disease progression, symptom management, medications, and the hospice philosophy.
• Communicate significant changes in patient conditions promptly to hospice and attending physicians.
• Ensure timely and accurate clinical documentation, including assessments and care plans.
• Participate in administrative and compliance activities, including workflow audits and survey readiness tasks.
• Maintain infection control standards and universal precautions.
• Provide leadership and guidance to field staff, conducting supervisory visits when necessary.
• Participate in ongoing training, updates, and required meetings.
• Foster a culture of compassion, professionalism, and teamwork in our organization.
• Perform additional duties as assigned.

Qualifications:
• Current RN license in the state of employment.
• Graduation from an accredited nursing program.
• At least one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Skills and Competencies:
• Strong understanding of hospice principles and Medicare/Medicaid regulations.
• Capability to conduct thorough patient assessments and create individualized care plans.
• Effective communication skills with patients, families, and multidisciplinary team members.
• Strong time management and prioritization abilities in dynamic environments.
• Proficient in using EMR systems, Microsoft Office, and mobile technologies.
• Ability to uphold confidentiality and support quality outcomes.
