Job Summary:
Provide exceptional nursing care according to established protocols and policies.
Key Responsibilities:
• Implement and monitor patient care plans while documenting and communicating patient conditions effectively.
• Coordinate all disciplines involved in patient care.
• Execute physician and nursing orders accurately.
• Assess and coordinate discharge planning needs with the healthcare team.
• Utilize knowledge and skills to identify and address patients' physical, psychosocial, cultural, spiritual, and age-related needs for personalized care.
• Delegate appropriate care to nursing personnel based on their expertise.
• Engage in daily interdisciplinary patient rounds and contribute relevant insights on patient conditions.
• Adhere to ANA Scope and Standards of Nursing Practice, Code of Ethics, and Social Policy Statement.
• Develop and revise individualized care plans as necessary.
• Participate in professional growth activities beyond patient care.
• Other duties as assigned.
Qualifications:
Candidates must have experience in educating patients on lifestyle changes to enhance cardiovascular health and prevent future heart issues. Familiarity with exercise equipment, patient data analysis, and collaboration with a multidisciplinary healthcare team is essential.
• Graduation from an accredited nursing school.
• Minimum of an Associate degree; BSN preferred.
• Current Connecticut RN license.
• BLS certification from the American Heart Association.
• Strong assessment and critical thinking skills.
• Experience in caring for patients recovering from heart conditions or related cardiovascular events.
