Ambulatory Registered Nurse Patient Navigator - Thoracic in Charleston, WV

**Overview:** We are seeking an experienced Ambulatory Registered Nurse Patient Navigator to join our team at Memorial-MSOB. As a Nurse Navigator, you will play a critical role in ensuring timely access and seamless coordination of services throughout the continuum of care for our thoracic patients. **Responsibilities:** • Assume leadership for the CAMC Thoracic/Esophageal Programs and drive the execution of strategic priorities • Cost-effectively manage patients across the entire continuum of care • Develop, implement, and evaluate clinical practice guidelines/care paths for patients • Coordinate patient care across all settings with all healthcare providers • Provide educational information and resources to patients to assist with coping, foster self-care activities, and assure high-quality value-added care • Collaborate with internal and external resources to develop and maintain a telemedicine and remote monitoring network • Evaluate clinical, quality, and economic outcomes of patient care • Collect and evaluate data related to patient population to provide information for ongoing quality improvement • Monitor clinical outcomes and system issues related to quality care delivery • *Qualifications:** • Associate's Degree • Registered Nurse (Required) • Must demonstrate competency through an initial orientation and ongoing competency validation to independently perform tasks and additional duties as specified in the job description • *Benefits:** • Full-time regular 1.0 position • Varies work schedule • Location: Memorial-MSOB, Charleston, WV • Competitive salary and benefits package
