Job Description
A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.
Key Responsibilities
Evaluate member needs and recommend care plans to ensure optimal healthcare outcomes
Coordinate with providers and community resources to address members' unmet needs
Monitor member progress and update care plans as necessary to meet goals
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
2 - 4 years of related experience in care management
Active RN - Registered Nurse state licensure or compact state licensure
Ability to work remotely while residing in Arizona
