Department: Skilled Nursing *

- Provide basic bedside care to long term care patients under the direction of a Registered Nurse (RN) or physician, functioning within scope of license. * Contribute to the assessment of patients by collecting data for analysis by a Registered Nurse (RN). * Educate patients/families about disease treatment plan including self-care post discharge, holistic health needs, available resources and follow up care. * Assure resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate.
Licensure / Certification / Registration: * Licensed Practical/Vocational Nurse obtained prior to hire date or job transfer date required.
High School diploma equivalency OR 1 year of applicable cumulative job specific experience required. * Full-time, Days * Every other weekend required As part of our Independent Living, Assisted Living, Short-term Rehabilitation, Memory Support or Long-term Care teams, you will be empowered to provide compassionate, personalized care and develop lasting relationships with our residents and their loved ones.
