About the position NorthKey Community Care is seeking a full-time Behavioral Health Nurse RN to engage and assist in the care of behavioral health care clients in the outpatient setting. This position will be at our Madison location in Covington, KY, and will also go between our Florence, KY office. Work hours are generally Monday-Friday from 8:30 a.m. - 5:00 p.m. NO WEEKEND, EVENING, and HOLIDAYS!! Flexible work schedule! QUALIFICATIONS Licensed by the Kentucky nursing board based on appropriate nursing education, training, and experience. Relevant knowledge, training, and experience. JOB DUTIES Assist physicians and nurse practitioners in the care of patients as part of medication management as well as in the development of service plans based upon assessment data. Assist physicians and nurse practitioners conducting hospital discharge clinics for medication assessments and prescribing as indicated by patients' presenting issues. Conduct routine screenings for the need of additional therapeutic and physical health services. Provide consultation to other staff as indicated. Complete quality documentation of services provided, including service plans, progress notes, medication informed consent, treatment plan reviews, and other required information to assure compliance with organizational policies and third-party compliance standards. Ensure patients are seen routinely prior to providing medication refills. Adhere to appropriate standards of care. Assist physicians and nurse practitioners in monitoring individuals on caseload for changes or decline in mental status or medical conditions. Collaborate with other staff members to promote quality patient care. Participate in interdisciplinary team communications. Handle medication orders and medications by following policies and relevant state regulations and quality standards. Ensure appropriate communication with other internal and external professionals for effective delivery of quality patient care.
