Bilingual Registered Nurse, - Home Health
Make an everyday difference as a CenterWell Home Health nurse by offering individualized, one-on-one care that assists patients in regaining their independence in the comfort of their homes. Collaborate closely with a dedicated team of healthcare professionals to create and manage care plans that foster recovery and help patients return to their cherished activities.
Home Health Registered Nurse Responsibilities
• Conduct admission, case management, and follow-up skilled nursing visits for patients receiving home health services.
• Provide ongoing care and case management for patients, with follow-up as directed by the Clinical Manager.
• Collaborate with the physician to create the initial treatment plan, using the physician's orders and initial patient assessment.
• Execute hands-on care, evaluate the care plan, and educate the patient according to physician instructions under the supervision of the Clinical Manager.
• Update the plan as needed in consultation with the physician based on ongoing evaluations and as required by policy/regulation.
• Coordinate necessary care, involving various healthcare personnel such as Physical Therapists, Occupational Therapists, and Home Health Aides.
• Continuously report patient care, condition, and progress to the patient's physician and Clinical Manager.
• Implement the care plan in collaboration with the patient and family to achieve the resolution of needs/problems.
• Direct/supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written personal care instructions that align with the current care plan.
• Ensure proper documentation completion by aides/personal care workers as part of the supervisory responsibility.
• Discharge patients after consulting with the physician and Clinical Manager, preparing and completing necessary clinical documentation.
• Prepare accurate medical documentation including case conferences, contacts, medication changes, re-certifications, progress updates, and care plan adjustments.
• Prepare visit/shift reports, update/summarize patient records, and collaborate with other healthcare disciplines for optimal patient care.

Required Experience/Skills
• Diploma, Associate, or Bachelor Degree in Nursing
• At least one year of nursing experience preferred
• Strong background in medical-surgical, ICU, ER, or acute care
• Home Health experience is advantageous
• Current and unrestricted Registered Nurse license
• Current CPR certification
• Strong organizational and communication abilities
• Valid driver’s license, auto insurance, and reliable transportation
Compensation
• $45.00 - $63.00 per visit/unit
• $70,500 - $96,900 annually base pay
Scheduled Weekly Hours
The salary range provided denotes a fair estimate of the starting base pay for full-time (40 hours per week) employment at the time of the posting. The pay range might fluctate based on geographic factors and individual salary will differ according to demonstrated job-related skills, experience, educational background, and certifications.
