A company is looking for a Bilingual Transition of Care Nurse.

Key Responsibilities

Outreach to caregivers and patients after hospital discharge for Transition of Care visits

Coordinate care between patients and their PCP / specialists post-discharge to ensure continuity of care

Provide anticipatory guidance and education to families while assessing barriers to care

Required Qualifications

Bachelor's in nursing from an accredited university required

Compact Licensed RN, or eligibility to register for other state licenses

Minimum of 2 years direct clinical experience in Emergency Department / Triage, Complex Care, or PICU preferred

Experience with pediatric population required

Bilingual in English and Spanish preferred
