Job Description
A company is looking for a NICU Care Manager (RN).
Key Responsibilities
Develops and facilitates complex care management activities for members, focusing on personalized care plans
Coordinates care between members, families, and healthcare providers to ensure appropriate services are received
Monitors member status and updates care plans as necessary to meet individual goals and needs
Required Qualifications
Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
2 - 4 years of related experience
RN - Registered Nurse - State Licensure and / or Compact State Licensure required
