Key Responsibilities
Assess member needs across various health domains and coordinate care plans
Connect clients to necessary social services and advocate on their behalf
Maintain documentation and report on program compliance and client progress
Required Qualifications
Active California Licensed Vocational Nurse (LVN) license
2+ years of experience in care management or related fields
Working knowledge of government and community health resources
Willingness to work Monday-Friday with potential evening and weekend hours
Bilingual (English / Spanish) preferred
