Job Description
A company is looking for an LVN Case Manager.
Key Responsibilities
Assess member needs across various health domains and coordinate care plans
Connect clients to necessary social services and advocate on their behalf
Maintain patient records and complete documentation within established timeframes
Required Qualifications
Active California Licensed Vocational Nurse (LVN) license
2+ years of experience as a care manager or related role
Knowledge of government and community resources related to social determinants of health
Willingness to work Monday-Friday with possible evenings and weekends
Bilingual (English / Spanish) preferred
