Job Responsibilities: • Provide skilled nursing care to patients in various clinical settings under provider direction. • Utilize nursing processes to assess, plan, intervene, and evaluate patient care. • Collaborate with providers to ensure continuity of care and respond to patient needs. • Conduct comprehensive assessments of physical and psychosocial needs in patients and their families. • Create and implement nursing care plans based on patient evaluations. • Communicate effectively with providers regarding patient status and recommendations. • Collect and document important health information accurately and assist in scheduling visits. • Ensure patient records are complete and coordinate with providers on outstanding issues. • Independently carry out nursing actions based on assessments and identified problems. • Manage prescription calls to pharmacies according to provider instructions and monitor patient responses. • Maintain thorough documentation of assessments, interventions, and progress in patient care. • Triages patient calls with precision and professionalism, implementing appropriate interventions. • Provide empathetic support to patients and families, respecting their privacy and confidentiality.
