Position Overview:
As a Registered Nurse under the direction of a provider, you will deliver skilled nursing care in various clinical settings. Utilizing the nursing process, you will assess, plan, intervene, and evaluate patient responses to health issues, adhering to established practices and protocols. You will report directly to a Practice Manager.
• Collaborate with providers to deliver comprehensive office-based nursing care.
• Assess physical, psychosocial, and environmental factors affecting patients and families; formulate and implement effective nursing care plans.
• Maintain open communication with providers regarding patient assessments and needs.
• Initiate actions, including cross-training, to ensure seamless patient care.
• Collect and document patient health status and medical histories while assisting in scheduling office visits.
• Review patient records for completeness and address outstanding issues with providers.
• Demonstrate independent nursing judgment based on assessments and identified problems.
• Under provider direction, manage prescription calls to pharmacies and monitor patients' responses to medications.
• Maintain accurate documentation of patients' ongoing assessments, progress, and responses to care.
• Triage telephone inquiries, demonstrating prompt and professional nursing interventions.
• Support patients and their families with empathy, maintaining confidentiality and respect for privacy.
Qualifications:
• Must possess a current Connecticut RN License.
