About this role as a Care Navigator Registered Nurse (RN) The Care Navigator RN is the primary HopeHealth representative within assigned hospitals, nursing homes, assisted living facilities, physician practices, health centers, and the wider community. This role plays a crucial part in promoting HopeHealth's hospice and palliative care services, driving growth, and organizational reputation. The position also collaborates in transitioning patients to the appropriate HopeHealth service line and serves as a subject matter expert in all services offered by the organization. This position primarily covers a territory in Rhode Island. The ideal candidate is passionate about working in serious illness care, is compassionate and an avid learner with a strong work ethic. What will you do as a Care Navigator Registered Nurse (RN) • Cultivates and develops relationships in assigned hospitals, nursing homes, assisted living facilities, physician offices, health centers and the community at large to drive HopeHealth’s hospice and palliative care growth in assigned territory. • The Care Navigator RN is a subject matter expert in all services offered by HopeHealth including hospice, palliative care, bereavement support, caregiver support, dementia services and community education. • Presents hospice and palliative care services to patients, families and referral partners in a clear and complete manner highlighting service benefits and eligibility. • Completes goals of care conversations, hospice informational sessions and hospice evaluations. • Works in close collaboration with hospice and palliative care and the Access Center to provide best-in-class customer service, and transition patients to the appropriate care. • Submits accurate, relevant and timely documentation in the EMR as required and appropriate. • Arranges care coordination meetings and follows up on service recovery issues. • Leverages and fully understands use of CRM to identify opportunities for growth. Documents in the CRM territory activity, tasks and results on an ongoing and timely basis. • As requested, provides professional and consumer education on hospice and palliative care services . • Seeks out and attends professional and consumer conferences to further develop relationships and drive organizational growth. • Completes other tasks as requested by supervisor. As needed, covers needs in other territories. • Consistently applies HopeHealth values in all aspects of work, complying with established mission, policies, procedures and objectives in quality assurance, safety, environment, infection control and HIPAA compliance. What are the qualifications to be a Care Navigator Registered Nurse (RN) • To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. • The requirements listed below are representative of the knowledge, skill, and/or ability required. . • RN required; Bachelor of Science Degree in Nursing preferred; five plus years of nursing experience. Hospice, palliative care or experience working with seriously ill patients preferred but not required. • Dual RN license in RI and MA required. • Valid driver’s license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.
