Role Description We are seeking an experienced RN Case Manager for a full‑time, on‑site assignment in Cedar Rapids, IA. This role focuses on coordinating patient care plans, facilitating discharge planning, collaborating with interdisciplinary teams, and ensuring efficient transitions of care. The ideal candidate is organized, clinically strong, and passionate about patient advocacy.
