RN Home Health Case Manager – Milwaukee North (West Allis, WI)

40 hrs/week | 8-hour Days | Jan 12–Apr 11, 2026

This contract is for a nurse who’s already confident in field-based home health and can handle a steady caseload with strong independence. It follows a classic Monday–Friday daytime schedule with no call or weekends, which keeps the workflow pretty predictable.

Shift & Schedule
• 8-hour day shift
• Monday–Friday, 8:00 a.m.–4:30 p.m.
• No on-call
• No weekends
• Holiday coverage as needed
• May float or assist other home health teams

Core Requirements

You’ll need to come in ready to operate as an independent Home Health Case Manager:
• At least 1 year of recent Home Health RN experience
• Strong OASIS proficiency
• Solid patient education skills
• Comfortable managing Start of Care visits (goal: 3/week)
• Strong wound care experience:
• Wound vacs
• Pleural drains
• Ostomy care
• Able to perform:
• Peripheral blood draws
• Central line care
• IV administration (elastomeric and pump-based systems)
• Excellent assessment and documentation skills
• Reliable transportation, active driver’s license, and auto insurance

Licensing & Certifications
• Active RN license (WI or compact)
• Active BLS (AHA)
