Position Description: The RN Case Manager provides skilled nursing care to patients in their home. This individual is responsible for coordinating care with the interdisciplinary team, patient, family and referring agency, and assumes responsibility for the coordination of care. The RN Case Manager provides service in accordance with the Agency philosophy, standards and policies. Essential Functions: • Initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate timeframes. • Prepare clinical assessment documentation and progress notes within a timely manner. • Assess patient’s condition, initiate plan of care, re-evaluate and update as necessary. • Effectively communicate with ancillary and clinical staff to maintain consistency and continuity of patient care, convey current information to the transferring facility including MAR and notification of transfer to the patient’s primary physician. • Adequately assess and reassess pain, and utilize appropriate pain management techniques. Educate the patient and their family regarding pain management. • Demonstrate knowledge of medications and their correct administration based on patient's age and clinical position. • Obtain all necessary physician orders and collaborates with physician as patient’s needs dictate. • Initiating appropriate preventive and rehabilitative nursing procedures. • Follow up with obtaining lab results and notify physician of any significant results according to the Agency’s Policy and Procedures. • Re-evaluate the need for continued care on an ongoing basis; initiate recertification every 60 days. • Plan for discharge from the Agency with the patient and caregiver. • Promote patient/family/caregiver education using various verbal and written communication techniques that take patient’s/family’s cultural, ethnic and/or personal needs or preferences. • Supervise Home Health Aides every other week according to State, Federal and Agency requirements. • Supervise Licensed Practical Nurses every 30 days according to State, Federal and Agency requirements. • Initiate needed referrals to other disciplines and/or community resources. • Schedule staff and caseload effectively. Manage the assignment of appropriate personnel for their client. Professional Qualifications • Complies with all organizational policies, procedures and ethical business practices. • Maintains patient confidentiality and HIPAA compliance at all times. • Completes all required annual education, training, evaluations and maintains current knowledge and competency through continuing education. • Maintains all regulatory and licensure requirements. • Reports to work on time and completes assigned tasks within designated timeframes. • Practices according to professional nursing standards and the agency’s code of ethics. • Represents the organization in a professional and positive manner. • Communicates and supports the mission, values and goals of the organization. • Adheres to agency dress code and professional appearance. Regulatory Requirements: • Graduate of an accredited school of nursing, with a current state licensure as a Registered Nurse. • 2 years of experience as an RN, preferably in a home health care setting. • Current CPR certification, proof of TB PPD given within the past 12 months, and Hepatitis consent/declination. • Valid State driver's license and reliable transportation. Compensation and Benefits: The RN Case Manager is eligible for a $5,000 sign-on bonus. We offer a benefits package that includes healthcare, dental and vision coverage for employees. Additionally, we provide 401K matching options and paid time off. Expected Salary Range: $89,000 - $92,000 based on experience. Physical Demands: Reasonable accommodations may be provided to enable individuals with disabilities to perform essential job functions. The role requires frequent standing, walking, speaking, and handling objects, as well as regular use of medical equipment in patients’ homes. Occasional stooping, bending, reaching overhead, and assisted lifting of patients (50+ pounds) may be required.
