Key Responsibilities:
• Collaborate with an interdisciplinary team to develop and update individualized care plans.
• Conduct thorough assessments across various settings, including patients' homes and facilities.
• Provide clinical supervision to aides and volunteers, guiding quality care delivery.
• Administer specialized procedures such as medication management and venous access care.
• Offer emotional support during end-of-life moments, including bereavement visits.
• Maintain accurate and detailed documentation in compliance with policies.
• Participate in 24/7 on-call rotations and team meetings to ensure seamless patient care.
• Communicate regularly with physicians and staff to coordinate services and resources.
• Stay current on hospice best practices through ongoing professional development.
Required Skills & Qualifications:
• Valid Massachusetts RN license in good standing.
• Associate’s degree in Nursing; Bachelor’s degree preferred.
• Minimum of 3 years nursing experience, with 1+ year in an acute setting.
• Strong assessment and critical thinking skills.
• Compassionate, adaptable, and team-oriented mindset.
• Valid driver’s license with insured vehicle in good working order.
• Familiarity with hospice philosophy and principles.
