Position Highlights
Varied Shifts, Monday-Friday, 7:30am-4:00pm - can be flexible, occasional weekends (6/7 weekends/year, no evening/nights)

Overview
In this role, you will act as a bridge ensuring smooth transitions for patients moving from Med/Surg and medical ICU settings to appropriate care levels. Collaborate with physicians and payors to make the best use of resources, while maintaining continuity of care throughout the treatment process.
Key Responsibilities:
• Deliver outstanding customer service while maintaining productive relationships with patients, families, physicians, and community agencies. Ensure timely medical updates for outside facilities during hospital stays and at discharge.
• Collaborate with physicians and monitor documentation to ensure efficient, evidence-based care for patients of all ages throughout their hospitalization. Complete thorough medical record documentation promptly.
• Stay informed about services covered by managed care organizations, private insurances, and government payers.
• Coordinate all discharge planning for patient transfers, including to skilled nursing facilities, outpatient services, and home health care.
• Arrange necessary services such as home health care, outpatient therapy, durable medical equipment, and more for post-discharge needs.
• Participate in multidisciplinary rounds and patient care conferences as needed.
• Work with Patient Advocate Services to explore financial resources for patients.
• Maintain compliance with state-required paperwork for transfers to long-term care facilities and assist with Medicare discharge protocols.
• Contribute to the development of standard operating procedures and ensure compliance with regulatory standards.
Qualifications:

Required:
• Active Registered Nurse (RN) license in the state upon hire.
Preferred:
• Board Certified Case Management Nurse (CMGT-BC) through the American Nurses Credentialing Center (ANCC).
• 4-6 years of hospital experience or relevant education and experience.
