Case Manager Registered Nurse – Field – Must reside in North Miami Florida in North Miami Beach, FL

Position Summary: • Candidates must be located in Miami Dade County, specifically in the North Miami Area. • This role will require 50-75% travel for face to face visit with members in Miami Dade. • Schedule is Monday-Friday, 8:00am – 5:00pm, standard business hours. We are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Registered Nurse to join our Case Management team. Nurse Case Manager is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Nurse Case Manager will effectively manage a caseload that includes supportive and medically complex members. • Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration. • Nurse Case Manager provides clinical assistance to case managers when needed to determine appropriate services and supports due to member’s health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports. • Nurse Case Manager will provide training to CM staff as needed/requested (i.e., Fall Risks, Medications, Behavioral Health, etc.). • Qualified candidates must have the ability to multitask, prioritize and effectively adapt to a fast-paced changing environment. As well as effective communication skills, both verbal and written. Required Qualifications: • Bilingual, English and Spanish. • Active and unrestricted RN License in Florida. • Must reside in North Miami FL. • Willing and able to travel 50-75% of their time to meet members face to face in Miami Dade County and surrounding areas in FL. Preferred Qualifications: • Managed Care experience preferred. • Long Term Care experience preferred. • Case management and discharge planning experience preferred. • 2+ years of clinical experience required. • 1+ year(s) proficiency with computer skills which includes. navigating multiple systems required. Education: • Active unrestricted RN licensure in FL required. • Bachelors in Science and Nursing Preferred. Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $60,522.00 - $129,615.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.  Our people fuel our future.
