Job Duties and Responsibilities of Charge Nurse LPN: • Assess, plan, implement, and evaluate individualized care plans in collaboration with physicians and healthcare teams. • Monitor patient progress and respond promptly to changes in condition. • Supervise and support RNs, LPNs, and CNAs, providing mentorship and guidance. • Assign duties, manage schedules, and ensure adequate staffing coverage. • Maintain compliance with facility policies, procedures, and regulatory requirements. • Conduct audits and assessments to uphold quality standards and identify improvement opportunities. • Document and report patient care, staff performance, and incidents accurately and promptly. • Respond effectively to medical emergencies, coordinating with emergency teams as needed. • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans. Qualifications and Experience of Charge Nurse LPN: • Associate’s or Bachelor’s degree in Nursing from an accredited institution. • Valid LPN or RN license. • Minimum of 2 years of nursing experience, preferably in skilled nursing or long - term care. • Previous supervisory experience is a plus. • BLS certification required; ACLS certification preferred. • Strong leadership, organizational, and communication skills. • Ability to make sound clinical decisions and manage multiple priorities effectively.
