Position Summary
The Charge Nurse provides leadership and direct resident care on an assigned unit/shift in our skilled nursing facility. This role supervises nursing staff (including RNs, LPNs, CNAs, and other team members), ensures high-quality clinical care in compliance with state and federal regulations, and promotes a positive, resident-focused environment. The Charge Nurse serves as a key clinical resource, coordinates care, and responds to emergencies while maintaining excellent standards of practice.
Key Responsibilities
• Lead and supervise the nursing team during assigned shift, including delegation of tasks, staff assignments, and performance oversight
• Conduct resident assessments, monitor health status, and identify changes in condition; promptly notify physicians, family members, and leadership as needed
• Administer medications, treatments, and procedures in accordance with physician orders, facility policies, and scope of practice
• Develop, update, and implement individualized care plans in collaboration with interdisciplinary team (therapy, dietary, social services, etc.)
• Ensure accurate and timely documentation in electronic health records (EHR) and compliance with all regulatory requirements (CMS, Colorado Department of Public Health and Environment, etc.)
• Respond to emergencies, initiate appropriate protocols, and coordinate with emergency services when necessary
• Promote infection control, safety, and quality assurance practices; participate in audits and performance improvement initiatives
• Foster positive relationships with residents, families, and staff; address concerns and support a culture of compassion and respect
• Orient and mentor new staff members and provide ongoing education and support to the nursing team
• Act as a liaison between shifts, communicating key information to oncoming staff and leadership
Qualifications
• Required:
• Active, unencumbered Registered Nurse (RN) license in the state of Colorado
• Current CPR/BLS certification
• Strong clinical assessment and critical thinking skills
• Excellent communication, leadership, and organizational abilities
• Commitment to resident dignity, quality care, and regulatory compliance
• Preferred:
• 1–2+ years of experience as a nurse in skilled nursing, long-term care, or rehabilitation setting
• Prior charge nurse, supervisory, or leadership experience
• Knowledge of PointClickCare or similar EHR systems
• IV certification (for LPNs, if applicable)
