The Clinical Care Coordinator (RN) is a patient-facing, administrative-focused nursing role that supports care coordination and patient communication to ensure timely, organized, and high-quality care. This role manages clinical messaging while delivering a warm, professional patient experience. Clinical tasks such as injections or phlebotomy may be performed as needed to support the care team.Role SummaryThe Clinical Care Coordinator (RN) is a key clinical and patient-facing role responsible for supporting front-desk clinical operations, coordinating patient care, managing clinical communications, and ensuring timely follow-up of patient needs. Operating at the intersection of nursing and administrative care, this role ensures an exceptional patient experience and efficient clinic workflow.While the primary focus is administrative coordination and patient communication, this RN may also perform clinical tasks (such as injections, phlebotomy, or standard nursing visits) when needed to support team capacity.This role requires a professional, friendly, patient-oriented mindset, excellent communication skills, and strong clinical judgment to guide triage and escalation decisions.Key ResponsibilitiesCheck patients in and out with clinical accuracy and a welcoming demeanorSupport front-desk patient interactions, questions, and clinical needsEnsure smooth transitions between clinical and administrative workflowsManage patient clinical communications via patient portal and phoneTriage inbound messages and escalate urgent or complex concerns to providersPerform clinical callbacks for test result notifications and next-step explanationsSort and organize incoming clinical results for daily provider huddlesProactively identify overdue or abnormal results and escalate as neededUpdate prescriptions and communicate with pharmacies when necessaryAct as a care coordinator for patient inquiries, clinical needs, referrals, and follow-upPartner with providers and care team members to ensure continuity of careProvide standard nursing visits, injections, phlebotomy, and other clinical support as backupQualificationsMust-HaveActive Registered Nurse (RN) license in good standingMinimum 2+ years’ experience as a Registered NurseOutpatient, ambulatory, or primary care experienceStrong clinical judgment and triage skillsExcellent verbal and written communication skillsProfessional, warm, and patient-centered approachHighly organized with capacity to manage multiple clinical workflowsNice-to-HaveExperience in concierge medicine, Direct Primary Care (DPC), or membership-based carePrior experience in care coordination or clinical operationsFamiliarity with population health, results management, or preventive care workflowsTools & SystemsExperience using an Electronic Medical Record (EMR) — training provided; prior Cerbo experience is helpful but not requiredComfortable managing clinical communications via patient portals, secure messaging, phones, and related platformsAbility to learn and navigate clinical tracking systems, lab interfaces, and pharmacy communication toolsStrong general computer literacy with ability to quickly learn new softwareDepartment & ReportingDepartment: Clinical OperationsReports to: Practice ManagerWork Environment & ScheduleLocation: Fully in person at the Boulder officeSchedule:Monday–Thursday: 8:00am–5:00pmFriday: 8:00am–2:00pmWe value work-life balance and provide a supportive team environment to facilitate high-quality care and professional fulfillment.Compensation & BenefitsSalary Range: $45 - $53/hrBenefits:Paid Time Off (PTO)Paid HolidaysPaid Sick Leave401(k) opportunityDedicated professional, financial, and personal growth supportHow to ApplyTo be considered for the Clinical Care Coordinator (RN) role, please submit:Email: ava@cloudmedical.ioSubject: Clinical Care Coordinator (RN) Application – [Your Name]Required Materials:Resume or CVBrief cover letter describing your interest in the role and why you’re a great fitCurrent RN license/certification documentationCompany DescriptionCloud Medical is a modern direct primary care (DPC) clinic rooted in relationship-driven, data-informed, and prevention-focused care.
