Clinical Care Registered Nurse (CCRN) – Clinic / Home Health
Location: Battle Creek, MI
Duration: 13 Weeks Contract
Shift Timings: Days | Monday–Friday | 8:00 AM – 4:30 PM
Guaranteed Hours: 40 hours/week

Job Description:

Provide direct nursing care to participants in clinic and home-based settings.
Independently manage acute and chronic medical conditions in collaboration with Primary Care Providers (PCPs).
Triage emergent clinical situations and apply advanced nursing judgment.
Administer medications, treatments, and procedures as prescribed.
Conduct regular home visits including wound care, lab draws, nursing assessments, and follow-up care.
Utilize individualized care plans to ensure high-quality, patient-centered care.
Document all care accurately using the TruChart EMR system.
Collaborate effectively with licensed and non-licensed healthcare team members.

Requirements:
• Active Michigan RN License
• Minimum 1 year RN experience (frail elderly, geriatric, LTC, or similar population)
• Home Health experience preferred
• Wound Care experience preferred
• BLS/BCLS certification (AHA)

Job Type: Contract

Pay: $1,800.00 - $2,000.00 per week
