This position combines direct patient care with program development to support our educational endeavors.Personality CharacteristicsCritical ThinkerStrong multitaskerExcellent communicatorDetail-orientedProactive problem-solverCORE RESPONSIBILITIESClinical Program Development Design and implement comprehensive orientation programs for new nursing staffDevelop clinical pathways for high-acuity home health careCreate and maintain clinical education curricula for ongoing staff developmentEstablish programs and frameworksLead quality improvement initiatives and clinical outcome analysisDevelop clinical protocols for complex cases (tracheostomy care, IV therapy, wound management)Partnership & Education Serve as clinical liaison to hospital systems throughout Central OhioDevelop and deliver educational presentations to hospital discharge planning teamsCreate and facilitate "Learning" sessions on home health Build collaborative relationships with hospital case managers and clinical staffProvide consultation on complex discharge planning casesRepresent DHM Agency at healthcare professional meetings and conferencesDirect Patient Care & Case Management Provide skilled nursing visits to approximately 20 patients per monthConduct comprehensive assessments and develop individualized care plansManage complex, high-acuity cases requiring specialized nursing skillsCoordinate interdisciplinary care with physicians, therapists, and other healthcare providersComplete OASIS assessments, care plan updates, and clinical documentationServe as clinical resource for challenging patient situationsStrategic Leadership Participate in organizational strategic planning and clinical decision-makingAnalyze market trends and competitive landscape in home health servicesDevelop metrics and reporting systems for clinical quality and partnership successCollaborate with leadership on business development through clinical excellenceQUALIFICATIONSRequired:Master's degree in Nursing (MSN) preferred;
