Key Role Responsibilities:
Lead discharge planning and coordinate post-hospital care for patients within 30 days after discharge, collaborating with a multidisciplinary team to ensure the best outcomes.
Assess patient needs, determine appropriate levels of care, and facilitate smooth transitions to the next phase of recovery.
Work with insurance providers to secure coverage and support patient education and preventive care programs.
Play a key role in reducing readmissions and improving patient satisfaction through effective communication and planning.
Qualifications:
Requires an Associates of Nursing (ASN).
Bachelors of Nursing (BSN) preferred.
Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
Requires that the RN has graduated from a nationally accredited nursing program.
Requires 3-5 years of relevant experience.
Requires Basic Life Support certification through the AHA.
Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
Requires proficiency in Microsoft Office and applications.
Requires understanding of medical record requirements, regulations and policies.
