LOCATION: Stroger Hospital (Medical/Surgical - General Surgery/Orthopedics Unit 8SSHIFT: 7:00 A.M. - 7:30 P.M.SALARY: $39.462 - $53.481 hourlySalary is commensurate with years of experience indicated at time of application submittal. Experience not disclosed or documented at the time of application will not be considered for initial step placement. To provide direct patient care to patients 18 years old and up. Patients are admitted with complex medical conditions/diseases from the Emergency Department, PACU and ICU (Intensive Care Unit). We receive patients from Direct Transfers, Ambulatory Clinics, Hospital to Hospital Referrals and Direct Admissions from home.Common medical diagnosis/conditions and post-operative surgical conditions include but are not limited to Diabetes, Pneumonia, Chronic Obstructive Pulmonary Disease (COPD), Kidney Failure (Acute and Chronic), Congestive Heart Failure (CHF), Bowel Obstructions, Autoimmune Diseases, Sickle Cell Anemia, Blood Disorders, Drug/Alcohol Abuse and Sepsis.Common (Post-Operative) Surgical procedures may include Total Knee/Hip Replacements, Reconstructive Orthopedic conditions. In addition to General Surgery, Cardio/Thoracic, Vascular, Gynecology and Trauma Patients.JOB SUMMARY: The Clinical Nurse II is a professional care provider who is responsible and accountable for overall management of patient care on a specific shift as well as maintaining comprehensive knowledge of patient’s condition. Provides safe, effective and individualized care using the nursing process. Performance of these duties require skills, effective working relationships, setting priorities, and demonstrated knowledge of standard operating policies and procedures. Typical DutiesProvides general direction, guidance and coordination of patient care activities delivered by other levels of nursing staffMakes appropriate and timely decisions in a crisis of emergency situationWorks and communicates effectively with a variety of individualsApplies knowledge of standard operating policies and proceduresIdentifies, plan of care for the total needs of patients considering their age specific requirementsOperates safely clinical equipment necessary for providing patient careInitiates and performs cardiopulmonary resuscitation activitiesApplies the principles of Infection Control and Environment of CareCollects assessment date relative to the following factors; biophysical, psychosocial, cultural and spiritual values, environmental, self-care capabilities, safety , infection control, learning needs, and ability to manage continuing care needs after dischargePerforms reassessments according to divisional/unit standards and patient care needsPerforms transfer assessments when patients are transferred in or out of the unitEnsures completion of transfer assessmentsPerforms and ensures formulation and communication of an individualized plan of care derived from assessment data.Utilizes assessment data in formulating age specific plan of care consistent with: A) The therapies of other disciplines; B) Standards of nursing practiceInvolves patient/significant others, and collaborates with other health care team members in planning nursing careUtilized principles of growth and development across life span in formulating age specific needs of the patients: neonate/infant, child, adolescent, adult, geriatricEnsures collaboration and communication with patient/family and other health care team members are accomplishedEnsures implementation of the nursing plan of carePerforms interventions and ensures implementations are done in a competent, skillful and safe mannerEnsures that patient’s responses to interventions are evaluated and modified accordinglyEnsures provision of interventions to meet patient identified discharge needsEnsures education in provided to patient and family based on assessed learning needsEnsures accurate documentationEnsures the staff records pertinent informationRecords accurately, concisely and legiblySigns documentation with legible full signature and classificationEnsures comprehensive delivery of patient careAdheres to and ensures staff follows principles of: infection control, environment of careDemonstrates competence in the safe and effective use of clinical equipment and ensure staff does the sameEvaluates effectiveness of plan of care and revised interventions as appropriateDemonstrates leadership/management ability; competent in making patient care/staff assignmentsIdentifies staffing needs to ensure safe, quality patient careFacilitates timely admission of patients to the unitEnsures timely discharge of patientsPlans, coordinates team/unit care problems in a timely mannerFollows the chain of command in referring problemsDemonstrates and promotes teamwork and effective interpersonal relationships with all health care team members, patients and familiesDemonstrates knowledge of hospital-wide services and makes appropriate referrals as indicatedParticipates and facilitates: quality assessment and improvement activities, unit policy/procedures, unit rounds, patient care team, conferences, end of shift report, CPR/fire drillsDemonstrates awareness of patient/family rights and ethical/legal issuesEnsures patient/family confidentiality and privacy are maintainedTreats patient/family with respect and courtesyTakes appropriate actions address ethical concerns for self and staffParticipates actively in staff orientation and staff development activities for the unitPerforms as resource nurse/preceptor when necessaryFacilitates attendance of staff to scheduled educational activitiesDemonstrates responsibility and accountability for processional practice, growth and developmentMaintains certification as requiredSeeks opportunities to improve professional knowledge and skillsTakes accountability for accurate reporting of unusual occurrences, e.g., medication, errors, patient falls, etc.Renews professional license in a timely mannerAttends safety, infection control in-services and maintains all certificationsDemonstrates punctualityMaintains a good attendance recordAdheres to departmental dress code policyPerforms other duties as assigned MINIMUM QUALIFICATIONS: Must be licensed as a registered nurse in the State of Illinois is required.Must possess an active Basic Life Support certification (BLS) or able to obtain one with two (2) weeks of starting is required. (Must provide proof at time of interview if already obtained).Must possess a current Advanced Cardiovascular Life Support (ACLS) certification or able to obtain one with two (2) weeks of starting is required. (Must provide proof at time of interview if already obtained).Must possess at least One (1) year of current Medical/Surgical nursing experience within an acute care setting is required.PREFERRED QUALIFICATIONS:Bachelors of Science in Nursing (BSN) from an accredited college or university is preferred.
