Description of Position
Acts
as liaisons between medical staff and patients, their families, and
caregivers. Assigned as the point in contact for patients during hospital
stay and follow-up treatment post discharge. Provides customized and
continuous care with utilization of health resources while adhering to internal
and external regulations and requirements.
Qualifications and Requirements
Graduate of an accredited school of nursing,
with current Registered Nurse licensure in the State of Hawaii required. Associates Degree in Nursing (AND) or a Bachelor’s in Nursing (BSN)
required. Certification in Care Coordination, Case Management,
Utilization Management, Quality Management, and / or related field
preferred. Requires recent experience within the past two years in care
coordination, case management, quality management, and / or utilization
management; or any combination of education and experience which would provide
the necessary knowledge, skills and abilities to meet the minimum
qualifications to perform the essential functions of this position. Requires
current certification for Cardiopulmonary Resuscitation (CPR) at Basic Life
Support (BLS) level.
Starting Rate $119,567
