As a Clinical Registered Nurse, you will play a pivotal role in creating outstanding patient experiences from day one by helping to establish workflows, foster a collaborative care team, and set benchmarks for accessible, same-day care. In this impactful role, you will utilize your clinical expertise to triage patients, assist providers, and ensure effective care coordination throughout the health center. Your Responsibilities • Deliver same-day clinical support to patients through in-person and phone triage once the clinic is operational. • Employ nursing protocols and sound clinical judgment to assess urgent medical needs effectively. • Collaborate in developing and enhancing patient flow processes conducive to compassionate care. • Provide direct patient care while supervising medical assistants and LVNs within the Care Team. • Monitor follow-up care after patients return from emergency room visits or hospital discharges. • Work alongside the Population Health team to oversee chronic care management and overdue screenings. • Act as a clinical resource, mentor, and role model for best nursing practices. • Facilitate daily team huddles and manage provider inboxes to ensure continuity and high-quality care. • Administer and verify injections and vaccines as necessary. • Engage in ongoing quality improvement efforts, training initiatives, and cross-site support.
