Join our dedicated team at Notre Dame Hospice Care as a Clinical Team Lead. In this full-time position, you will play a vital role in providing comfort, compassion, and support to patients at the end of life and their families, ensuring they receive the highest quality of care.
The Clinical Team Lead is responsible for overseeing an interdisciplinary team that includes Nursing, Social Work, Spiritual Care, Home Health Aides, Music Therapy, and Massage Therapy. Your leadership will ensure that all care aligns with established standards, regulations, and organizational protocols. By fostering clinical excellence and collaboration within the team, you will help provide a comprehensive, patient-centered approach to hospice care.
Key Responsibilities:
• Lead the hospice interdisciplinary team in delivering high-quality care.
• Ensure compliance with state and federal regulations, as well as organizational standards.
• Promote a collaborative and efficient team environment.
• Maintain timely and accurate patient documentation.
• Manage patient care with compassion and efficiency, focusing on quality of life and symptom management.
Qualifications:
• Graduation from an accredited nursing school; BSN preferred.
• Active RN license in Massachusetts.
• Minimum of five years of hospice nursing experience.
• CHPN certification preferred.
• Experience in hospice leadership or management is strongly preferred.
• Knowledge of hospice reimbursement guidelines and regulatory standards is desirable.
• Demonstrated leadership skills and the ability to work within a team effectively.
• Exceptional interpersonal and communication skills.
• Valid driver’s license and reliable transportation.
• Commitment to continuing education and best practices in hospice care.
