Job Summary
Caregiver Support Network Home Health and Hospice is looking for a part time or full time CNA in the Salt Lake City area. We pay 90% of health insurance premiums for employees and your family. Sign on bonus- $1,000 for working 32 hours per week paid after 3 months working. $500 for working 20 hours per week paid after 3 months working. Quarterly bonuses. The agency was founded in April 2000 to provide private duty services and therapeutic activity for people with Alzheimer’s disease and other dementia, at the same time focusing on supporting and educating their family caregivers. Our carefully chosen specialty programs help our clients return home sooner, recover faster and remain as independent as possible.
Responsibilities & Duties
• Observe self-administration of medications by reminding client to take medications and opening containers
• Assist the client in personal grooming, oral care, dressing and toileting.
• Light housekeeping, prepare meals and assist the client with eating.
• When specifically approved by CSN, transport client to appointments as scheduled.
• Take and record oral temperatures.
• Administer emergency CPR / first aide; contacting physician or 911 if situation requires further care.
• Provide or arrange for social interaction and recreational/diversional activities
• Document observations and services provided in the individual patient record at the time of visit.
• Complete time log accurately, clearly, timely and provide appropriate communication to Care Manager, and other support specialists as needed, about client / family issues and response to services provided.
• Demonstrate competency in all areas as outlined in Performance Review.
• Communicate with Care Manager weekly on client’s status.
• Comply with and support of company policies and procedures.
Qualifications & Skills
• At least 18 years of age
• Maintain current CPR certification
• Able to read instructions of supervisor and write observations in client record
• Clearly communicate verbally and in writing
• Must have own transportation to appointments
• Must have compassionate attitude
• Attend at least 12 hours of in-service per year
• Attend First Aid Training
