Job Summary Caregiver Support Network Home Health and Hospice is looking for a part time or full time CNA in the Salt Lake City area. We pay 90% of health insurance premiums for employees and your family. Sign on bonus- $1,000 for working 32 hours per week paid after 3 months working. $500 for working 20 hours per week paid after 3 months working. Quarterly bonuses. Our carefully chosen specialty programs help our clients return home sooner, recover faster and remain as independent as possible. Responsibilities & Duties • Observe self-administration of medications by reminding client to take medications and opening containers • Assist the client in personal grooming, oral care, dressing and toileting. • Light housekeeping, prepare meals and assist the client with eating. • When specifically approved by CSN, transport client to appointments as scheduled. • Take and record oral temperatures. • Administer emergency CPR / first aide; contacting physician or 911 if situation requires further care. • Provide or arrange for social interaction and recreational/diversional activities • Document observations and services provided in the individual patient record at the time of visit. • Complete time log accurately, clearly, timely and provide appropriate communication to Care Manager, and other support specialists as needed, about client / family issues and response to services provided. • Demonstrate competency in all areas as outlined in Performance Review. • Communicate with Care Manager weekly on client’s status. • Comply with and support of company policies and procedures.
