Community Health Nurse must be willing to travel to counties: Randolph, Delaware, Henry, Wayne, Grant, Madison, Hamilton with the possibility of travel outside of these counties

The Community Health Nurse improves health outcomes by providing both clinical support and health education to staff, clients and community partners. The Fresh Start Connect Program specifically serves pregnant and post-partum persons with substance use disorder. They facilitate communication between clients, their families, and the medical community in order to improve maternal health outcomes. This role is instrumental in providing post-partum and pregnancy education and support as part of an interdisciplinary team. This position requires travel and home visitation with clients. This position is 85% travel within the DCS regions listed above and the mileage and travel expense will be reimbursed. The expectation of this position is that all client contact is face-to-face.

Applicants must have be licensed as a RN in the state of Indiana. Applicants with at least 1 year maternity experience required and substance use treatment preferred. First Aid & CPR certification is required and must be maintained. VOAOHIN will provide certification if not already obtained
