DescriptionNorthpointe is a Community Mental Health (CMH) organization providing services to individuals with serious mental illness, emotional disturbance, substance abuse disorders, and intellectual/developmental disabilities across Iron, Dickinson, and Menominee counties in the UP of Michigan. Northpointe itself was established in 1995 but has roots as a CMH dating back to 1964. Our presence can be found through remote emergency services, in school and home-based programs, and in clinical and residential home settings.
Job PurposeWe are looking for a passionate, driven individual to join our team as a Community Nurse serving Iron, Menominee, and Dickinson counties. This role is offered on a full-time hourly basis and does require on-the-job travel.
The Community Nurse's primary function is to provide seamless, patient centered care for physical, behavioral, and social needs to vulnerable individuals with intellectual/developmental disabilities and/or serious mental illness who reside in Northpointe owned and operated Adult Foster Care (AFC) group homes and those enrolled in the Behavioral Health Home program. The Community Nurse works as a part of a multi-disciplinary integrated care team within our community to improve outcomes, access, and efficiency.

Employees will be required to follow any other job-related instructions and to perform any other job-related duties requested by the supervisor.Essential DutiesProvides nursing services through person-centered planning practices that include medication monitoring, assessment, planning, implementation, and evaluation of individuals at risk or those experiencing psychiatric/physical symptoms. Services will vary with medical orders.Works effectively with Residential Home Managers and other members of the treatment team in a collaborative approach by actively participating in the Home staff meetings to ensure effective communication.Demonstrates the ability to educate individuals and staff related to health and medications using current information and best practice guidelines when applicable.•Maintains current knowledge by completing required continuing education hours and by seeking educational opportunities to expand knowledge base.Focuses on integrated care and assists the Residential Home Manager with coordination of health care services for individuals within the residential system while attending all medically necessary appointments.Completes follow-up documentation on all medical orders for residents and assures that all residential home staff are adequately trained to manage any health emergencies.Identifies health/nursing care needs and implements care plans/procedures as needed for all patients under the direction of the Integrated Care Team Lead.Educates staff within the residential and community supports to implement health care plans/procedures/protocols for individuals, ie Seizure protocols for each resident.Trains staff and oversees the medication administration process within residential services.Reviews Residential medication and fall related unusual incident reports and determines necessary interventions.Provides consultation to residential care staff regarding health-related issues for individuals who reside in residential setting.Assists the Director of Community Inclusion and Home Managers with Enhanced Pharmacy benefits and Durable Medical Equipment for individuals residing within the residential homes.Assists with the site reviews of contracted residential locations.Assists the Director of Community Inclusion and Home Managers with preparing for Residential inspections including but not limited to: NorthCare, MDHHS, CARF, AFC licensing, MIOSHA, and the fire marshal.Assists the Director of Community Inclusion and Home Managers with preparation for surveys and follows up with requested plan of corrections.Communicates with medical providers, long term care and hospitals regarding records including admission/discharge at the direction of the Integrated Care Team Lead.RequirementsQualificationsThis position will actively support the mission, vision, and values of the agency.The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Education, Certifications, and/or ExperienceGraduate of an accredited school of nursing.Current Michigan license to practice as a licensed practical nurse (LPN) Two to three years of practical experience in a variety of settings and experience in the area of community health preferred experience working within a Community Mental Health setting preferred.Must have a valid driver’s license.Must meet state qualifications of QIDP or QMHP within 1 year of hire.
