Compact LPN Tele-Triage Nurse in Honolulu, HI
Job Description
A company is looking for a Tele-Triage Licensed Practical Nurse (LPN).
Key Responsibilities
Monitor assigned phone lines and triage symptom-based calls according to approved protocols
Develop and implement care plans, document recommendations, and coordinate services with acute care settings
Conduct assessments, ensure compliance with requirements, and participate in performance improvement activities
Required Qualifications
Degree / certification from an accredited school of Nursing
At least one year of nursing experience
Current licensure as a LPN in the state of practice with a compact license
Clinical experience with individuals with intellectual and developmental disabilities or related disorders
One year of home health experience preferred
